I grant permission for Monroe County Community School Corporation to request personal and employment references from those listed and certify that I can, and will upon request, substantiate all statements made on this application and that such statements are true, complete to the best of my knowledge and are made in good faith.

I give permission for MCCSC to obtain a copy of my motor vehicle record at any time during my employment with MCCSC. I understand that transporting students in my personal vehicle may be a requirement of my employment.
